Wellness Program

Earn Your 2026 Incentive Funding If you are enrolled in the Company HSA or HRA medical plan, you may choose to participate in UIC’s 2026 wellness program. Personify Health is an award winning wellness platform that has been designed to cultivate good lifestyle habits. With Personify Health, you can learn how to improve in areas that are important to your health. This new program will assist you in reaching your health goals and even work on those goals with friends, if you choose to do so. You can monitor your activity, sleep, mood, and more. Other features include coaching and ways to reduce stress and eat healthy.

To earn your 2026 wellness incentive funding, you and your spouse (if applicable) must complete the two requirements below on the Personify Health website by December 31, 2026. Please note that employees enrolled in the Basic medical plan do not qualify for wellness incentive funding.

Wellness incentive requirements:

1. Complete Online health check survey AND

2. Earn 5000 points through activity of your choice or receive and log a preventive care service on the Personify Health website. For the Health Assessment, log into the Personify Health website, then navigate to Home > Rewards.

Employee-only $425 contribution to your HSA or HRA Employee-plus-one or more $850 contribution to your HSA or HRA
