Personify Health Wellness Program

If you’re enrolled in the Company HSA or HRA medical plan, you’re eligible to join UIC’s 2025 wellness program through Personify Health, an award-winning platform designed to build healthy habits. With tools to track your activity, sleep, mood, and more, Personify Health helps you reach your health goals—solo or with friends! It also offers coaching and tips on reducing stress and eating healthier.

Earn UIC Rewards

To qualify for your 2025 wellness incentive, both you and your spouse (if applicable) must meet these two requirements by December 31, 2025:

1. Complete the online health check survey.
2. Earn 5,000 points through activities or a preventative care service.

Employees on the Basic medical plan are not eligible for the incentive.

Incentive Funding

Once both requirements are met, you’ll see your incentive within 90 days:

• $400 contribution to your HSA or HRA (Employee-only)
• $800 contribution to your HSA or HRA (Employee + one or more)
